What is SF-50?


Standard Form 50, Notification of Personnel Action, or SF-50, is a form by the Office of Personnel Management (OPM). SF 50 is used to document personnel actions and employment events.


 


How to fill out SF-50?


Fill out SF-50 accurately and truthfully to avoid any problems. SF-50 has several copies with the same fillable fields. The following are the recipients for the copies:


  • Copy 1 — Employee Copy

  • Copy 2 — OPF Copy - Long-Term Record

  • Copy 3 — Payroll Copy

  • Copy 4 — Chronological Journal File Copy

  • Copy 5 — Utility Copy

  • Make sure to answer all the items before giving the copies to the different recipients


     


    Item 1 — Name


    Enter the full legal name of the employee, as it appears on all his or her government records, in the following format: Last Name, First Name, Middle Name.


     


    Item 2 — Social Security Number


    Enter the Social Security number (SSN) of the employee.


     


    Item 3 — Date of Birth


    Enter the date of birth of the employee.


     


    Item 4 — Effective Date


    Enter the effective date of the action.

    Item 5-A — Code


    Enter the code required by the chapter that explains how to process the action.


     


    Item 5-B — Nature of Action


    Enter the nature of the action for the code shown in Item 5-A.


     


    Item 5-C — Code


    Enter the primary authority code.


     


    Item 5-D — Legal Authority


    Enter the regulation used for the nature of the action.


     


    Item 5-E — Code and Item 5-F — Legal Authority


    Items 5-E to 5-F are used only when a second authority is required for the nature of the action.


     


    For Item 5-E, enter the code for the second authority.


     


    For Item 5-F, enter the regulation used for the nature of the action.


     


    Item 6-A — Code to Item 6-F — Legal Authority


    Items 6-A to 6-F are used when a second action with the same effective date is processed on the same SF-50. Use the First Action section as a guide to complete the items.


     


    Item 7 — FROM: Position Title and Number


    Enter the position title and number shown in the “To” portion of the employee’s last Notification of Personnel Action or the position the employee is leaving.


     


    Item 8 — Pay Plan


    Enter the pay plan shown in the “To” portion of the employee’s last Notification of Personnel Action or the position the employee is leaving.


     


    Item 9 — Occ. Code


    Enter the occupational code shown in the “To” portion of the employee’s last Notification of Personnel Action or the position the employee is leaving.


     


    Item 10 — Grade of Level


    Enter the grade or level shown in the “To” portion of the employee’s last Notification of Personnel Action or the position the employee is leaving.


     


    Item 11 — Step or Rate


    Enter the step or rate for the employee’s current salary.


     


    Item 12 — Total Salary


    Enter the total salary of the employee.


     


    Item 12-A — Basic Pay


    Enter the employee’s rate of basic pay.


     


    Item 12-B — Locally Adj


    Enter the difference between Item 12C and 12A. You may leave this blank if the employee is not entitled to administratively uncontrollable overtime pay, availability pay, or supervisory differential.


     


    Item 12C — Adj. Basic Pay


    Enter the maximum adjusted rate of basic pay after taking into account all applicable pay caps.


     


    Item 12D — Other Pay


    Enter the total amount of other allowances or differentials that are excluded from the amounts shown in Items 12 and 12D.


     


    Item 13 — Pay Basis


    Enter the appropriate code for the basis on which the employee is currently being paid.


     


    Item 14 — Name and Location of Position’s Organization


    Enter the name and location shown in the “To” portion of the employee’s last Notification of Personnel Action.


     


    Item 15 — TO: Position Title and Number to Item 22 — Name and Location of Position’s Organization


    Items 15 to 22 require the same details from Items 7 to 14 but in the context of the position an employee is going to, the job he or she will be doing.

    Item 23 — Veterans Preference


    Enter the appropriate code based on preference for the appointment, adverse action, reduction in force, or performance-based action purposes. The following are the codes:


  • 1 — None

  • 2 — 5-Point

  • 3 — 10-Point/Disability

  • 4 — 10-Point/Compensable

  • 5 — 10-Point/Other

  • 6 — 10-Point/Compensable/30%

    Item 24 — Tenure


    Enter the appropriate code for tenure. The following are the codes:


  • 0 — None

  • 1 — Permanent

  • 2 — Conditional

  • 3 — Indefinite

    Item 25 — Agency Use


    This item is only completed when required by an agency.


     


    Item 26 — Veterans Pref. for RIF


    Mark YES to confirm employees Preference for Reduction in Force (RIF); otherwise, mark NO.


     


    Item 27 — FEGLI


    Enter the appropriate Federal Employees Group Life Insurance (FEGLI) code.


     


    Item 28 — Annuitant Indicator


    Enter the appropriate code and title for Annuitant Indicator.


     


    Item 29 — Pay Rate Determinant


    Enter the Pay Rate Determinant code.


     


    Item 30 — Retirement Plan


    Enter the code for Retirement Plan.


     


    Item 31 — Service Comp. Date (Leave)


    Enter the Service Computation Date (Leave). Enter in the following format: Month-Day-Year.


     


    Item 32 — Work Schedule


    Enter the appropriate code for the Work Schedule.


     


    Item 33 — Part-Time Hours Per BiWeekly Pay Period


    This item is only used if the employee works part-time hours.

    Item 34 — Position Occupied


    Enter the code for the position occupied. The following are the codes:


  • 1 — Competitive Service

  • 2 —Excepted Service

  • 3 — SES General

  • 4 — SES Career

    Item 35 —  FLSA Category


    Enter the code for the Federal Labor Standards Act (FLSA) category the employee is under. The following are the codes:


  • E — Exempt

  • N — Non-exempt

    Item 36 — Appropriation Code


    Enter the Appropriation Code.


     


    Item 37 — Bargaining Unit Status


    Enter the Bargaining Unit Status.


     


    Item 38 — Duty Station Code


    Enter the Duty Station Code.


     


    Item 39 — Duty Station


    Enter the Duty Station. Enter the city, country, and state or overseas location.


     


    Items 40 — AGENCY DATE to 44 are applicable to the HR office.


     


    Item 45 — Remarks


    Enter additional remarks.


     


    Items 46 to 50 requires authentication and information on approval by the HR office.


     


    Item 46 — Employing Department or Agency


    This is prefilled with “Department of Veterans Affairs.”


     


    Item 47 — Agency Code


    Enter the Agency Code.


     


    Item 48 — Personnel Office ID


    Enter the Personnel Office ID Number.
