What is Optional Form 306?

Optional Form 306 or OF 306, Declaration for Federal Employment, is a form by the U.S. Office of Personnel Management (OPM) used to determine an individual’s acceptability for Federal and Federal contract employment and enrolment status in the Government’s Life Insurance program.

How to fill out Optional Form 306?

Answer all the information required in Optional Form 306 accurately and truthfully. Providing any false statement in any part of this form or attachments to this form may result in you not getting the job or getting fired after you begin work. Moreover, you may be punished by a fine or imprisonment.

Item 1 FULL NAME

Enter your full legal name in the following format: First Name, Middle Name, Last Name, Suffix.

If you have only initials in your name, enter them and indicate “Initial only.” If you do not have a middle name, enter “No Middle Name.” If you are a “Jr.,” “Sr.,” etc., enter your Suffix.

Item 2 SOCIAL SECURITY NUMBER

Enter your unique nine-digit Social Security Number (SSN).

Item 3 PLACE OF BIRTH

Enter your place of birth, including the city and state or country.

Item 3b ARE YOU A U.S. CITIZEN?

Mark “YES” if you are a U.S. citizen; otherwise, mark “NO.” If you answered “NO,” provide your country of citizenship in the space provided.

Item 4 DATE OF BIRTH

Enter your date of birth in the following format: MM/DD/YYYY.

Item 5 OTHER NAMES EVER USED

Enter other names you previously used, such as your maiden name and nickname.

Item 6 PHONE NUMBERS

Enter your day and night phone numbers, including the area code.

Selective Service Registration

Item 7a

Mark “YES” if you are a male born after December 31, 1959; otherwise, mark “NO.” If you answered “NO,” proceed to Item 8.

Item 7b

Mark “YES” if you have registered with the Selective Service System; otherwise, mark “NO.” If you answered “NO,” proceed to Line 7c.

Item 7c

If you answered “NO” in Line 7b, describe your reason/s in Item 16.

Military Service

Item 8

Mark “YES” of you have ever served in the United States military? Otherwise, mark “No.” If you answered “YES,” list the branch, applicable dates, and type of discharge for all active duty. If your only active duty was training in the Reserves or National Guard, answer “NO.”

Background Information

Item 9

Mark “YES” if during the last 7 years, you have been convicted, imprisoned, on probation, or on parole. This includes felonies, firearms or explosives violations, misdemeanors, and all other offenses. If not, mark “NO.” If you answered “YES,” use Item 16 to provide the date, explanation of the violation, place of occurrence, and the name and address of the police department or court involved.

Item 10

Mark “YES” if you have been convicted by a military court-martial in the past 7 years. If not, mark “NO.” If you answered “YES,” use Item 16 to provide the date, explanation of the violation, place of occurrence, and the name and address of the military authority or court involved.

Item 11

Mark “YES” if you are currently under charges for any violation of the law. If not, mark “NO.” If you answered “YES,” use Item 16 to provide the date, explanation of the violation, place of occurrence, and the name and address of the police department or court involved.

Item 12

Mark “YES” if during the last 5 years you have been fired from aby job for any reason, quit after being told you would be fired, leave any job by mutual agreement because of specific problems, or were debarred from Federal employment by the Office of Personnel Management or any other Federal agency. If not, mark “NO.” If you answered “YES,” use Item 16 to provide the date, explanation of the problem, the reason for leaving, and the employer’s name and address.

Item 13

Mark “YES” if you are delinquent on any Federal debt. This includes delinquency arising from Federal taxes, loans, overpayment of benefits, and other debts to the U.S. government, plus defaults of Federally guaranteed or insured loans, such as student and home mortgage loans. If not, mark “NO.” If you answered “YES,” use Item 16 to provide the type, length, and amount of the delinquency or default, and the steps that you are taking to correct the error or repay the debt.

Item 14

Mark “YES”  if any of your relatives work for the agency or government organization to which you are submitting Optional Form 306. This includes your father, mother, husband, wife, son, daughter, brother, sister, uncle, aunt, first cousin, nephew, niece, father-in-law, mother-in-law, son-in-law, daughter-in-law, brother-in-law, sister-in-law, stepfather, stepmother, stepson, stepdaughter, stepbrother, stepsister, half brother, and half sister. If not, mark “NO.” If you answered “YES,” use Item 16 to provide the relative’s name, relationship, and the department, agency, or branch of the Armed Forces for which your relative works.

Item 15

Mark “YES” if you receive or ever applied for retirement pay, pension, or other retired pay based on military, Federal civilian, or District of Columbia Government service. If not, mark “NO.”

Provide the information requested in Items 7 through 15 and 18c. If there are to be attached documents along with this form, provide your name, SSN, line number. Also, include the ZIP codes in all addresses provided.

Items 17 to 17b

Certify Optional Form 306 by signing and dating it. If you are applying for a position and have not yet been selected, read Item 17 and complete Item 17a. If you are being appointed, read Item 17, complete Item 17b, read item 18, and answer Items 18a, 18b, and 18c as appropriate.

Item 18a

Provide the date when you left your last Federal job in the following format: MM/DD/YYYY.

Item 18b

Mark “YES” if you waived Basic Life Insurance or any type of optional life insurance when you worked for the Federal Government the last time. If not, mark “NO.” You may also mark “DO NOT KNOW.”

Item 18c

If you answered “YES” to Item 18b, answer this item. Mark “YES” if you later canceled the waiver/s. If not, mark “NO.” You may also mark “DO NOT KNOW.” If you answered “NO,” use Item 16 to identify the type/s of insurance for which waivers were not canceled.
