What is Optional Form 306?


Optional Form 306 or OF 306, Declaration for Federal Employment, is a form by the U.S. Office of Personnel Management (OPM) used to determine an individual’s acceptability for Federal and Federal contract employment and enrolment status in the Government’s Life Insurance program.


You may receive Optional Form 306 during your employment process.

Item 1 FULL NAME


Enter your full legal name in the following format: First Name, Middle Name, Last Name, Suffix.


 


If you have only initials in your name, enter them and indicate “Initial only.” If you do not have a middle name, enter “No Middle Name.” If you are a “Jr.,” “Sr.,” etc., enter your Suffix.


 


Item 2 SOCIAL SECURITY NUMBER


Enter your unique nine-digit Social Security Number (SSN).


 


Item 3 PLACE OF BIRTH


Enter your place of birth, including the city and state or country.

Mark “YES” if you are a U.S. citizen; otherwise, mark “NO.” If you answered “NO,” provide your country of citizenship in the space provided.

Enter your date of birth in the following format: MM/DD/YYYY.


 


Item 5 OTHER NAMES EVER USED


Enter other names you previously used, such as your maiden name and nickname.

Mark “YES” if during the last 7 years, you have been convicted, imprisoned, on probation, or on parole. This includes felonies, firearms or explosives violations, misdemeanors, and all other offenses. If not, mark “NO.” If you answered “YES,” use Item 16 to provide the date, explanation of the violation, place of occurrence, and the name and address of the police department or court involved.

Mark “YES” if you have been convicted by a military court-martial in the past 7 years. If not, mark “NO.” If you answered “YES,” use Item 16 to provide the date, explanation of the violation, place of occurrence, and the name and address of the military authority or court involved.


 


Item 11


Mark “YES” if you are currently under charges for any violation of the law. If not, mark “NO.” If you answered “YES,” use Item 16 to provide the date, explanation of the violation, place of occurrence, and the name and address of the police department or court involved.


 


Item 12


Mark “YES” if during the last 5 years you have been fired from aby job for any reason, quit after being told you would be fired, leave any job by mutual agreement because of specific problems, or were debarred from Federal employment by the Office of Personnel Management or any other Federal agency. If not, mark “NO.” If you answered “YES,” use Item 16 to provide the date, explanation of the problem, the reason for leaving, and the employer’s name and address.


 


Item 13


Mark “YES” if you are delinquent on any Federal debt. This includes delinquency arising from Federal taxes, loans, overpayment of benefits, and other debts to the U.S. government, plus defaults of Federally guaranteed or insured loans, such as student and home mortgage loans. If not, mark “NO.” If you answered “YES,” use Item 16 to provide the type, length, and amount of the delinquency or default, and the steps that you are taking to correct the error or repay the debt.

This includes your father, mother, husband, wife, son, daughter, brother, sister, uncle, aunt, first cousin, nephew, niece, father-in-law, mother-in-law, son-in-law, daughter-in-law, brother-in-law, sister-in-law, stepfather, stepmother, stepson, stepdaughter, stepbrother, stepsister, half brother, and half sister. If not, mark “NO.” If you answered “YES,” use Item 16 to provide the relative’s name, relationship, and the department, agency, or branch of the Armed Forces for which your relative works.

Mark “YES” if you receive or ever applied for retirement pay, pension, or other retired pay based on military, Federal civilian, or District of Columbia Government service. If not, mark “NO.”
