What is Form DS-82? 

 

Officially the U.S. Passport Renewal Application for Eligible Individuals, Form DS-82 is a passport renewal application form to apply for a new U.S. passport due to expiration. To be eligible for a passport renewal, all of the following conditions must be true: 

 
  • Your U.S. passport has your current name. 
    •  
  • Your U.S. passport was issued in the past 15 years. 
    •  
  • Your U.S. passport was issued when you were 16 years old or older. 
    •  
  • Your U.S. passport is undamaged and can be submitted with your application. 
    •  

    If one of the mentioned conditions is not met, you will need to apply for a new passport. 

     

    How to Fill Out Form DS-82? 

     

    Filling out the two-page Form DS-82 is simple, only requiring that you provide all the necessary information correctly to avoid any problem with your application for a renewal passport. 

    Start by selecting the document you are applying for — U.S. Passport Book, U.S. Passport Card, or both.

    Then provide the required information:

  • Name asks for your full legal name as it appears on your citizenship document.

  • Date of Birth asks for your date of birth following the MM/DD/YYYY format. 
    •  
  • Sex asks for your gender. 
    •  
  • Place of Birth asks for your place of birth regardless if it is in the U.S. or another country. 
    •  
  • Social Security Number asks for your SSN. Enter zeros if not applicable. 
    •  
  • Email asks for your active email address. 
    •  
  • Primary Contact Phone Number asks for an active phone number. 
    •  
  • Mailing Address asks for your mailing address, which may be used to send your passport. 
    •  
  • Names you have used asks for your previous legal names if applicable. 
    •  


  • Passport Book and/or Passport Card Information asks for your name as printed on your most recent U.S passport book and/or passport card, passport book and/or passport card number, and passport book and/or passport card issue date.

  • Name Change Information asks for information regarding your name change if applicable. Use this section if your current name is different from the name you have on your last U.S. passport book and/or card.

  • Name of Applicant asks for your full legal name as it appears on your citizenship document.

  • Date of Birth asks for your date of birth in MM/DD/YYYY format.

  • Height asks for your height in feet and inches or centimeters. 
    •  
  • Hair Color asks for your natural hair color. 
    •  
  • Eye Color asks for your natural eye color.

  • Occupation asks for your current job, if applicable and you are 16 years old or older.

  • Employer or School asks for the business name of the employer or name of the educational institution you go to if applicable. 
    •  
  • Additional Contact Phone Number asks for another phone number if you have any. 
    •  
  • Permanent Address asks for your permanent address if it is different from your mailing address. 
    •  


  • Emergency Contact asks for the information of the person who will be contacted in the event of an emergency. This person should not be traveling with you. 
    •  
  • Travel Plans asks for your set travel plans if applicable. Provide the departure and return dates and the countries you will visit. 
    •  

    What are the required documents? 

     

    Submit your most recently issued U.S. passport book and/or U.S. passport card. If the name you are currently using is different from the name on your most recent U.S. passport, provide a certified marriage certificate or court order showing the change of name. 
    Aside from the required documents, you should also submit an updated photo of yourself taken within the last six months and the fees in the form of a personal check or money order payable to “U.S. Department of State”. The Department of State strongly discourages sending money. For U.S. passport book, pay $110, for U.S. passport card, pay $30, for both, pay $140.
