What is Form DS-64?

Officially the Statement Regarding a Lost or Stolen U.S. Passport Book and/or Card, Form DS-64 is a legal document that U.S. passport holders complete and submit to the U.S. Department of State to report a lost or stolen passport book and card and to start the replacement process.
By reporting to the U.S. Department of State that your passport got lost or stolen using Form DS-64, it will create a record in the Consular Lost and Stolen Passport System, making your lost passport invalid. This will protect you from fraud and avoid unlawful border crossing using your identity; thus, if your passport gets lost or stolen, it is your obligation to notify the designated authorities immediately.
