What is Form 4506-T?

 

Officially the Request for Transcript of Tax Return, Form 4506-T is an Internal Revenue Service (IRS) form that U.S. citizens use to request and receive a tax return transcript. It may also be used to receive a tax account transcript, record of account transcript, wage and income transcript, and verification of non-filing letter.

 

How to Fill Out Form 4506-T

 

Form 4506-T is a one-page form with two additional pages for instructions. Provide all the necessary information for the federal agency to find your records without difficulties and delays. Thus, double-check the form before submission to avoid any issues with your application.

 

Before filling out Form 4506-T, you need to learn the types of transcripts you can request. Take note that the IRS only allows one transcript request per form. If you need to request for two or more types of transcripts, complete one Form 4506-T per request.

 

Types of Transcripts:

 
  • Tax Return Transcript is only available for the current tax year and returns processed during the prior three years. It contains information from the following original return documents as filed:

  • Form 1040 series
  •  
  • Form 1065
  •  
  • Form 1120
  •  
  • Form 1120-A
  •  
  • Form 1120-H
  •  
  • Form 1120-L
  •  
  • Form 1120S
  •  
     
  • Tax Account Transcript is available for the current tax year and up to 10 prior years. It provides return type, marital status, adjusted gross income, taxable income, and payment types.
  •  
  • Record of Account Transcript combines the tax return and tax account transcripts. It is available for the current year and three prior tax years.
  •  
  • Wage and Income Transcript shows data from the following information returns:
  •  
     
  • Form W-2
  •  
  • Form 1099 series
  •  
  • Form 1098 series
  •  
  • Form 5498 series
  •  
     

    The current tax year information is not available until the year after it is filed with the IRS.

     
  • Verification of Non-Filing Letter is proof that you did not file a return for the year you requested, attesting that the IRS has no record of your following form:

  • Form 1040
  •  
  • Form 1040A
  •  
  • Form 1040EZ
  •  
     

    The current year requests are only available after June 15.

     

    Follow the following guide to answer your Form 4506-T accurately:

     

    Line 1a asks for your full legal name, as it appears on your returns.

     

    Line 1b asks for your Social Security Number (SSN) or Individual Taxpayer Identification Number (ITIN).

     

    Line 2a asks for the full legal name of your spouse, as it appears on returns, if applicable.

     

    Line 2b asks for the SSN or ITIN of your spouse, if applicable.

     

    Line 3 asks for your current address.

     

    Line 4 asks for the address you used on your last filed return.

     

    Line 5 asks up to 10 numeric characters that will serve as your unique customer file number to appear on the transcript. Answering this line is not required. In addition, do not use your SSN.

     


    Line 6 asks for your tax form number.

     

    Lines 6a, 6b, 6c are used to indicate the type of transcript you are requesting. Mark the appropriate checkbox. Mark checkbox 6a to request a Tax Return Transcript, 6b for Tax Account Transcript, or 6c for Record Account Transcript.

     

    Line 7 is used to request a Verification of Non-Filing document. Mark the checkbox, if applicable.

     

    Line 8 is used to request a transcript of your Form W-2 or any filed form in 1099, 1098, and 5498 series. Mark the checkbox, if applicable.

     


    Line 9 asks for the ending date of the year or period of tax years requested using the MM/DD/YYYY format.

     


    Signature of taxpayers asks for your signature and date to authenticate the form. If for a corporation, partnership, estate, or trust, provide your title. If applicable, provide your spouse’s signature and date of signing.
