What is Form I-90?

 

Officially the Application to Replace Permanent Resident Card, Form I-90 is a legal form from the Department of Homeland Security — U.S. Citizenship and Immigration Services (USCIS). This application form is used by lawful permanent residents and permanent residents in commuter status to apply for replacement or renewal of existing Permanent Resident Cards.

 

Conditional permanent residents may also use Form I-90 to apply for the replacement of an existing Permanent Resident Card; however, they may not use it to replace an existing Permanent Resident Card, regardless of any reason, that is expired or expiring within 90 days.

 

How to Fill Out Form I-90?

 

Form I-90 contains nine parts that you need to complete. Answer all questions accurately to avoid encountering problems with your application. Answer “NA,” unless otherwise stated, for all questions that do not apply to you. If your answer to a question requires a number response “0” or none, write “None” unless otherwise directed.

 

Part 1. Information About You

Provide your Alien Registration Number (A-Number).

Provide your USCIS Online Account Number, if applicable.

Provide your full legal name.

Mark the appropriate checkbox to answer if your name has legally changed since the issuance of your Permanent Resident Card.

If you entered the U.S with an immigrant visa, provide the destination in the U.S. at the time of admission and the port-of-entry where you were admitted to the U.S.
