What is Form DS 4194?
Form DS 4194, Request for Authentication Services is a document by the US Department of State used by citizens to request that their important foreign documents be validated and authenticated by the Authentication Office.
The Authentication Office is an agency responsible for verifying documents by issuing them under the Seal of the US Department of State. With this seal, these documents can be submitted to foreign countries with little to no conflict.
Before you fill out your DS 4194 Form, you must first review the documents that you want to be authenticated and check if there are additional requirements for the authentication. You must also see if the document requires an apostille or an authentication certificate.
An apostille is a certification of the place of origin of public documents. This kind of certification, however, can only be issued by and for countries that are a part of the Apostille Convention or the Hague Convention. An apostille, according to this convention is only needed if all of the following criteria are met:

If you are filling out this form on behalf of another individual or company, input yes. If you inputted yes, then the information in Section 1 should be for the individual or company you are representing. Otherwise, input no. If you input no, you are not required to fill out this section of the form.
