What is Form VTR-34?

 

Form VTR-34, also known as the Application for a Certified Copy of Title, is a Texas Department of Motor Vehicles form that must be completed by the vehicle owner if the Texas Certificate of Title is lost, destroyed, or mutilated. 

 

How to fill out Form VTR-34?

 

To complete the form, you will need information about the vehicle and copies of your government-issued ID. Input the required information correctly. 

 

Application Checklist 

 

This section includes a list of what is required to complete and submit the form. Check each box once you have paid the fee, included a copy of government-issued photo ID, signed the form, included the original, and signed release of lien. If you are applying on behalf of an entity or using a power of attorney, he or she must sign too.

 

Vehicle Information 

 

Provide the required information about the vehicle, such as vehicle identification number, year, make, body style model, title or document number, and Texas license plate number.

 

Recipient Information

 

Enter the information about the recipient of the Certified Copy of Texas Title, such as their full name, mailing address, email, and phone number.

 

Government Issued Identification 

 

Select the type of identification card that you will submit. For each ID, provide the state or country where it was issued, the ID number, and the expiration date. Provide a photocopy of each ID.

    Certification 

     

    In this section, you must provide your signature, print your name, and indicate the date of completion. Use black or blue ink only.

     

    Submission

     

    This form may be submitted by mail or walk-in.

     

    Mail it with the $2.00 fee, copy of government-issued ID(s), and any other documentation to:

    If you will submit via mail, wait for at least 15 days to receive your Certified Copy of Title. Additional documents submitted with this application must be original. The original documentation submitted with the application will be returned to you. Payment should be in the form of a personal check, cashier's check, or money order. Fees are non-refundable

     

    If you will submit in person, prepare a $5.45 fee, bring this form and any required documents to a TxDMV Regional Service Center. Applications should not be mailed to the locations below, except Wichita Falls.

    Tips

     
  • You must wait 30 days after the last title was issued if you want to apply for a Certified Copy of Title.
  •  
  • If the information contained in the motor vehicle record is needed, Form VTR-34 is not suitable for this purpose. In this case, they should complete Form VTR-275 (Request for Texas Motor Vehicle Information) to obtain a title history, or current or expired title and registration verification of a vehicle record.
  •  
  • If the vehicle is titled in the name of multiple individuals, each individual must sign this form, and each must provide a copy of their government-issued photo ID. 
  •  
  • If the vehicle is titled in the name of an entity or has a recorded lien, the agent representing the entity or lienholder must sign the application and provide a letter of signature authority on letterhead, printed business card, or employee ID connecting the signing agent to the entity and a copy of the agent’s government-issued photo ID. 
  •  
  • If a power of attorney (POA) is used, the above requirements apply for both the individual or entity granting the POA and the individual or entity being granted POA.

  • If you want to donate your car and you don't want to go through the process of obtaining a duplicate title, you will need this form, a copy of your driver’s license, a POA from each owner listed on the title, and a lien release, if applicable. If there is a lien on the title, the VTR-34 must be signed by the lien holder and include a copy of the signing agent’s driver’s license and a business card or letter of authorization from the lienholder for the signing agent. When you have filled out the forms just keep them until the towing agent comes by to pick up the vehicle.
