    If there is another change, enter the reason for it. It may be a new license class, a wrong date of birth, or another valid reason.


     


    Social Security Number


    Enter your unique nine-digit Social Security Number (SSN).


     


    Address Where You Get Your Mail


    Enter the full address where you get your mail. Include Street Number and Name, Rural Delivery, and/or box number. If P.O. box, you must also answer the “Address Where You Live” item.

    Has Your Mailing Address Changed?


    Mark “Yes” if your mailing address has changed; otherwise, mark “No.”


     


    Has the Address Where You Lived Changed?


    Mark “Yes” if the address where you lived changed; otherwise, mark “No.”


     


    If you answered yes to either of the last two questions above, then addresses on all vehicle registrations tied to your ID number will also be updated with this address, unless you mark the appropriate checkbox under the “Has Your Mailing Address Changed” and “Has the Address Where You Lived Changed” items.


     


    If you are registered to vote, your voter registration record will be updated when you complete and submit Form MV-44. If you do not want your new address on your voter registration record, mark the appropriate checkbox under the “Has Your Mailing Address Changed” and “Has the Address Where You Lived Changed” items. If you do not check the box, your new address will be sent to the Board of Elections of your country of residence.


     


    Veteran Status


    Mark the box if you would like to have “Veteran” printed on the front of your document. If you mark the box, you must present proof that indicated an honorable discharge from military service.


     


    New York State Organ and Tissue Organization


    Mark “Yes” if you would like to enroll in the New York State Donate Life Registry and provide your Signature and the date when you signed. You may also mark “Skip This Question.”


     


    In addition, if you want to make a $1 voluntary donation to the Life...Pass It On Trust Fund for organ and tissue donation research and outreach, you may mark the appropriate checkbox.


     


    Voter Registration Questions


    Mark “YES — Complete Voter Registration Application Section” if you are not registered to vote where you live now and would like to apply to register; otherwise, mark “NO — I Decline to Register/Already Registered.”


     


    The following questions must be completed for ALL license /permit transactions.


    Mark “Yes” if applicable; otherwise, mark “No.”


     


    1. Has your driver’s license, learner permit, or privilege to drive a motor vehicle been suspended, revoked, or canceled, or has your application for a license been denied in this state or elsewhere, in the name you provide on this form or any other name? If “Yes” has your license, permit, or privilege been restored, or has your application been approved?


    2. Have you received treatment, do you currently receive treatment, do you take medication for any condition that causes unconsciousness or unawareness? If you marked “Yes,” you must submit Form MV-80U.1, even if you were released from the Medical Review Program.


    3. Do you need a hearing aid and/or full view mirror to drive a motor vehicle?


    4. Have you lost the use of a leg, arm, hand, or eye?


    4a. If you need to renew your driver’s license and you marked “Yes,” did this occur since your last driver’s license.


    4b. If you marked “No” to 4a, has your condition gotten worse since your last driver’s license?


     


    Parent/Guardian Consent


    Mark the appropriate box to determine the application. You may select:


  • Junior License

  • Non-driver ID Card (under 16).

  • If you are applying for either, provide your Signature, Relationship to the Applicant, and the Date of signing.


     


    Teen Electronic Event Notification Service (TEENS)


    If you would like to enroll in the TEENS program, enter ID Number on NYS Driver’s License, Permit, or Non-Driver ID Card of Consenting Parent or Guardian.


     


    Commercial Driver License Applicants Only


    Mark “Yes” if, in the past 10 years, a driver’s license was issued to you from another state in the U.S. or the District of Columbia; otherwise, mark “No.”
