What is the Maryland Change of Address Form?


Form VR-154, Application for Maryland Change of Address, or the Maryland Change of Address Form, is a state-level form by the Maryland Motor Vehicle Administration (MVA). Residents of the state of Maryland who are eligible to use it submit it to the said department to change their address on their Maryland’s driver license.


 


Residents of Maryland may also use the form to apply to register to vote in the state and change their political party. To register to vote, you must:


  • be a Maryland resident

  • be at least 16 years old — You may register to vote if you are at least 16 years old; however, you cannot vote unless you will be at least 18 years old by the next General Election.

  • not have been found by a court to be incapable of voting by reason of mental disability

  • not have been convicted of buying or selling votes

  • not have been convicted of a felony, or if you have, you have completed serving a court-ordered sentence of imprisonment

    The Maryland Motor Vehicle Law requires that you notify the Motor Vehicle Administration regarding any change in your address within 30 days of the change.

    How to fill out the Maryland Change of Address Form?


    The Maryland Change of Address Form contains four sections:


  • Section A — All Applicants

  • Section B — Voter Registration

  • Section C — All Applicants Information

  • Section D — Voter Registration Information

  • Residents who will use the Maryland Change of Address Form will need to answer sections A and D.


     


    The Maryland Change of Address Form is a two-page document, but you only need to answer the first page — the page that contains sections A and D. Complete Form VR-154 accurately and correctly to avoid any problems or delays with your request to change your address. Do not deliberately provide any false information as doing so is unlawful and may result in penalties or imprisonment or both.


     


    Section A — All Applicants


    This section consists of fields where you need to enter your information regarding your Maryland change of address.


     


    Driver’s License / Soundex Number


    Enter your driver’s license number. If you do not have a driver’s license, enter your Soundex number instead. A Soundex number code consists of a letter and three numbers.


     


    Date of Birth
    Enter your date of birth.


     


    Name


    Enter your full legal name in the following format: First Name, Middle Name, Last Name, Title (Jr., Sr.).


     


    Old Resident Address


    Enter your old resident address, including City, County, State, and Zip.


     


    New Resident Address


    Enter your new resident address, including City, County, State, and Zip.


     


    Title Number


    Enter your title number.


     


    Tag Number


    Enter your tag number.


     


    Sticker Number


    Enter your sticker number.


     


    Full Signature


    Affix your signature. Signing means that you certify, under penalty of perjury, that all statements made on your Maryland Change of Address Form are true and correct to the best of your knowledge, information, and belief.

    Section B — Voter Registration


    This section is used by applicants who want to apply to register to vote in Maryland or to change their political party affiliation.


     


    Item 1


    Mark the appropriate box if you do not want your voter registration to be updated. Take note that if you are already registered to vote in Maryland, the information in Section A will automatically update your voter registration information.


     


    Only complete items 3, 9, 13, and 14 if you are already registered to vote in Maryland and you only want to change your party affiliation.


    Only complete items 2 to 14 if you are not registered to vote in Maryland and would like to apply to register to vote. You may only register to vote if you are a U.S. citizen.

    Mark “Yes” if you are at least 16 years old; otherwise, mark “No.”


    Mark “Yes” if you are a U.S. citizen; otherwise, mark “No.”


     


    Take note that if you answered “No” to any of the questions in Item 2, you are not eligible to complete this form.

    Mark the appropriate boxes to determine your application type. You may select:

  • Address Change

  • Party Affiliation Change

    Mailing address


    Enter your mailing address if it is different from the residence address in Section A.

    Enter your date of birth in the following format: Month, Day, Year.

    Mark the appropriate box to determine your gender. You may select:

    Enter your daytime phone number.


     


    Item 8


    Enter your Social Security Number (SSN). Entering your SSN is voluntary. Your SSN will only be used for registration and other administrative purposes. It will be kept confidential.

    Mark the appropriate box to determine the political party you want to be affiliated with. You may select:


  • Democrat

  • Republican

  • Libertarian

  • Green

  • Unaffiliated (independent of any party)

  • Other — Specify the political party.

  • You must check only one box. Registering with a political party will allow you to take part in your political party of choice’s primary election, caucus, or convention.

    Mark the box if you need help at the polls.


     


    Item 11


    Mark the box if you would like to be an election judge.

    This item contains statements that you must agree with before signing the form.

    Enter your address on your last registration, if applicable, including Street Number, Street Name, City or Town, State, and ZIP code.


     


    Relevant information about using the Maryland Change of Address Form:


  • Your application must be received by a Maryland election office no later than 9 PM, 21 days before an election.

  • Your Voter Notification Card will be mailed to you if your qualification was completed and you were qualified.

  • Do not submit the form to any individual other than an official, employee, or agent of a County Board of Elections.
