If you currently have a mailing address that is separate from your current physical address, you will be asked to indicate this in the following line. For your mailing address, you will be asked to indicate in which city and state that mailing address is located. Indicate the corresponding ZIP code as well. Finalize all this information by placing the date of signing, once again with no specific format, as well as your signature in the indicated space.
Section 3
As previously stated, you are allowed to change the address of up to two (2) other individuals who are also requesting the same address change shown above.
For each resident, he or she must indicate his or her full legal name, followed by his or her former name if they had previously undergone the process of legal name change.
In the following line, he or she must write down his or her Idaho driver’s license number or identification card number. Following this, he or she must write down his or her daytime phone number, or the number used to contact them between the hours of 9:00 AM and 5:00 PM. Finally for this line, the additional resident is asked to write down his or her date of birth in no particular format.
He or she must also list down the license plate numbers of up to four (4) cars currently registered under his or her name. He or she must also indicate whether they wish to make changes to the physical address, mailing address if separate from their current residential address, or both. To finalize everything, the date of signing must be written down as well as the signature of the additional resident.
