TheI Idaho DMV Change of Address form, otherwise known as ITD 3239, is a form submitted to and used by the Idaho Transportation Department. What do I need to fill out the Idaho DMV Change of Address form?

To fill out this form, you will need to input all information that is required from you in this form.

Specifically, you will be needing your full, legal name, your former name if you have legally changed your name beforehand, your Idaho driver’s license number or identification card number, your daytime phone number, and your date of birth. You will also be asked to provide the license plate number of up to four (4) vehicles currently in your possession.

You will also be needing your current physical address, otherwise known as your current residential address, the city in which it is located, and the ZIP code. You will also need to indicate your mailing address if it is different from your physical address. For this, you will need the city, state, and ZIP code of your mailing address. You will also be asked to verify this information by inputting the date of signing as well as affixing your signature in the designated space.

Additionally, if there are any more residents in your household who also wish to change their address, then you may place all of their information in the appropriate space.You will have to write down his or her full legal name, former name if he or she has previously changed their name beforehand, Idaho driver’s license number, daytime phone number, date of birth, license plate number for up to four (4) vehicles currently in his or her possession, the date of signing and whether he or she wishes to change his or her current physical address, mailing address, or both. Finally, his or her signature must be placed within the designated space. A single Idaho DMV Change of Address form can account up to two (2) additional residents to a change of address.

Section 2

This section asks for your updated residential and mailing addresses as well as additional information regarding these.

The first line asks you to indicate your current physical address, otherwise known as the address in which you currently reside. You will have to indicate in which city in Idaho you live in. Additionally, you will have to write down the corresponding ZIP code. 

If you currently have a mailing address that is separate from your current physical address, you will be asked to indicate this in the following line. For your mailing address, you will be asked to indicate in which city and state that mailing address is located. Indicate the corresponding ZIP code as well. Finalize all this information by placing the date of signing, once again with no specific format, as well as your signature in the indicated space.

Section 3

As previously stated, you are allowed to change the address of up to two (2) other individuals who are also requesting the same address change shown above.

For each resident, he or she must indicate his or her full legal name, followed by his or her former name if they had previously undergone the process of legal name change.

In the following line, he or she must write down his or her Idaho driver’s license number or identification card number. Following this, he or she must write down his or her daytime phone number, or the number used to contact them between the hours of 9:00 AM and 5:00 PM. Finally for this line, the additional resident is asked to write down his or her date of birth in no particular format. 

He or she must also list down the license plate numbers of up to four (4) cars currently registered under his or her name. He or she must also indicate whether they wish to make changes to the physical address, mailing address if separate from their current residential address, or both. To finalize everything, the date of signing must be written down as well as the signature of the additional resident.
