What is DA 31 Form?

 

Officially the Request and Authority for Leave, DA 31 Form is a Department of the Army form. In general, members of the U.S. service military use the form to request a temporary leave from their post to attend to a family emergency or to have a vacation.

 

In the U.S., soldiers are entitled and can request for up to 30 days of leave per year. A soldier must submit a completed DA 31 Form to his or her commander at least 29 days before the start date of leave. Before completing and submitting the form, a soldier may inform his or her commander first to get approval. Once the leave is approved, a soldier on leave should carry the form with him or her during the entirety of the temporary time-off.

 

The form requires information that a soldier needs to provide; thus, his or her leave’s approval will depend on the supplied details. Neglecting to provide important and required information could lead to the request being denied.

 

How to Fill Out DA 31 Form?

 

DA 31 Form has four parts. A soldier applying for a temporary leave will need to provide personal and leave-related information. Providing specific details regarding the leave increases the chances of the request to be approved. On the other hand, an incomplete form may lead to the rejection of the request. When answering the form, a soldier should keep in mind to answer it as honestly and accurately as possible to avoid problems.

Box 1 may be left blank. It requires the request’s control number.

 

Part I requires personal and leave information.

 

Box 2 asks for your full legal name as it appears on your records. Follow the required format.

 

Box 3 asks for your Social Security Number (SSN).

 

Box 4 asks for your rank.

 

Box 5 asks for the date of leave application.

 

Box 6 asks for the address where you will stay during the vacation.

 

Box 7 asks for the type of leave to take.

 

Box 8 asks for your origin station and phone number.

 

Box 9 asks for the number of days of your leave.

 

Box 10 asks for the dates of your leave.

 

Box 11 asks for your signature.

 

Box 12 asks for your supervisor’s recommendation, whether approved or disapproved.

 

Box 13 asks for the approving authority’s signature and title.

 

Box 14 asks for the date and time of your departure and the name, title, and signature of the departure authority.

 

Box 15 is used to request a leave extension, if applicable. This section asks for the number of additional days, date approved, and the name, title, and signature of the approval authority.

 

Box 16 asks for the date and time of your return and the name, title, and signature of the return authority.

 

Box 17 asks for remarks, including chargeable leave dates, if applicable.

 

 

Part II is only used for an emergency leave request.

 

Box 18 contains leave-related information.

 

Box 19 will have instructions for scheduling return transportation.

 

Box 20 asks for the departed unit.

 

Box 21 asks for arrived Aerial Port Of Debarkation (APOD).

 

Box 22 asks for arrived Aerial Port Of Embarkation (APOE), for return only.

 

Box 23 asks for arrived home unit.

 

 

Part III is only used when traveling with dependents.

 

Box 25 asks whether you require a one-way or round trip. This section also asks for the names of dependents, your relationship with them, dates of birth for children, and their passport number.

 

 

Part IV is to be used by the person who authorizes your travel. You do not need to answer this part.
